Arizona Medical License
For residents who plan on moonlighting, becoming a chief, or seeking full-time employment in Arizona after graduation, you will need an Arizona medical license. For DOs, the application processing time is usually less than three months. However, for MDs, the processing can take as long as six months, so plan accordingly. Your chiefs recommend applying in November.
Requirements:

Copy of birth certificate, passport, or proof of immigration status

Headshot photograph of passport quality

Photo taken less than 60 days prior to date of application.

Medical school certification

An official copy of your medical school transcripts, copy of diploma, or letter with an official letterhead that confirms successful completion.
