Applying for Your Arizona License
For residents who plan on moonlighting, becoming a chief, or seeking full-time employment in Arizona after graduation, you will need an Arizona medical license. For DOs, the application processing time is usually less than three months. However, for MDs, the processing can take as long as six months, so plan accordingly. Your chiefs recommend applying in November.
All applicants will need:

Copy of birth certificate or passport (or proof of immigration status)

Headshot photograph of passport quality (photo taken less than 60 days prior to date of application). ~$15 at the post office.

Medical school certification (An official copy of your medical school transcripts, copy of diploma or letter with an official letterhead that confirms successful completion.) Cost varies by school - $5-10.

Step scores sent directly to the Board (with USMLE, it costs $70. You are offered two copies.

$500 non-refundable application fee (and later a $500 licensing fee once your application is approved)

Certification of identification notarized by Robin MacDermott (included in the application packet)

Attestation form notarized by Robin MacDermott (included in the application packet)

Post graduate training certification (The form is included with the application, must be filled out by Arletta and submitted directly to the Board by her.)

7. After the application is approved, you'll need to get fingerprinted using the fingerprint packet the Board sends to your mailing address. This can be done at a local police station or post office. You'll need to provide the fingerprint tech a check or money order for $50 made out to "Arizona Medical Board". The techniciant is required to fill out and date the identity verification form, place it with the fingerprint card and check or money order in an envelope, and seal and sign the envelope flap before returning the fingerprint card to you to be mailed.
